The Chronic Kidney Disease Clinic at Parkland Health & Hospital System was established in February, 2003 with the purpose of improving renal and cardiovascular outcomes in patients with kidney disease. Patients are referred from the hospital outpatient clinics, the outpatient Community-Oriented Primary Care Clinics, as well as clinics and practices in the community.
The clinic is staffed by a Nephrologist and a Physician Assistant both of whom evaluate patients referred to the clinic. During the initial visit, a complete history and physical exam, including ascertainment of cardiovascular risk factors, is performed. The Nephrologist and PA then recommend modification to the patient's medical management based on clinical practice guidelines promulgated by the National Kidney Foundation, American Heart Association, and American Diabetes Association.
Our clinic adopted a primary goal of slowing the progression of Chronic Kidney Disease by aggressive management of each patient's contributory risk factors.
