The Parkland Palliative Care team provides care to patients and their caregivers who are facing a life threatening illness. Parkland physicians refer patients to the Palliative Care program. The Palliative Care team focuses on physical, psychosocial, spiritual and emotional care and comfort. Bereavement care is also provided for caregivers for a least one year, which includes memorial services four times a year.
A physician, a nurse case manager, a social worker and a chaplain direct patient care activities. The Palliative Care team works very closely with Pharmacy, the American Cancer Society and a variety of community organizations. Hospice care is coordinated with several community agencies for eligible patients.
Volunteers also support Parkland's Palliative Care activities. Services are provided to inpatients at Parkland or as outpatients in the Palliative Care Clinic. The goal is to allow patients to remain in the comfort of their home as much as possible. The team maintains frequent telephone contact with patients and caregivers to accomplish this.
